Just a friendly reminder to bring your shot cards & school, sports, or camp forms with you to your physical or well-child visit.  There is no charge for forms brought in at the time of physical or well-check. 

Please note that due to privacy laws, we cannot fax school forms or immunizations cards. 

If dropping off a school form to be filled-out, please allow 10 business days for provider completion. 

For lost shot cards & school forms brought in 24 hours after the well-check, there will be a small fee.
